Real Doctors. Real Signatures. Verifiable Credentials.

SwiftCareMD is an online telehealth platform where every doctors note is reviewed and signed by a board-certified, U.S.-licensed physician. Every note includes the physician's full name, medical license number, NPI, and signature — credentials your employer or school can verify directly.

What Makes a Doctors Note Legitimate?

Employers and schools verify doctors notes by checking for specific credentials. Every SwiftCareMD note includes:

  • Physician's full name and medical degree (MD or DO)
  • State medical license number
  • National Provider Identifier (NPI) — verifiable in the CMS NPI registry
  • Practice name and contact information
  • Physician signature

How Asynchronous Telehealth Works

Asynchronous telehealth means you don't need a live video call or phone appointment. Instead, you complete a secure intake form describing your symptoms — and a licensed physician reviews your information on their own time. This is the same standard of care used by major health systems and the VA.

Every physician on our platform is board-certified, U.S.-licensed, and independently verifiable through the national NPPES registry.
